Mechanism of Action
Essential electron carrier in the mitochondrial electron transport chain (Complex III). Also functions as a potent lipid-soluble antioxidant, protecting membranes from oxidative damage.
Side Effects & Safety
Well-tolerated. Rare mild insomnia, GI upset, or headache. May reduce the effectiveness of warfarin — on case-report evidence, so uncertain rather than dismissible. Statin users are commonly deficient.

Interactions
Warfarin and other vitamin-K antagonists
May reduce the anticoagulant effect. CoQ10 is structurally similar to vitamin K, which gives a plausible mechanism, and a case report described a patient whose warfarin responsiveness fell while taking CoQ10 and returned after stopping it. Do not start or stop CoQ10 while on warfarin without arranging INR monitoring

Statins
Statins lower endogenous CoQ10 synthesis, which is the usual reason for supplementing alongside them. This is a rationale for use, not a hazard - but it does not by itself establish that supplementation resolves statin-associated muscle symptoms

Safety in pregnancy and breastfeeding has not been established in adequate trials. Not recommended without medical advice.
Who should avoid / see a doctor
Anyone on warfarin or another vitamin-K antagonist should treat CoQ10 as a change to their anticoagulation and clear it with the clinician who manages their INR. The evidence here is two case reports from 1998, not a trial - which makes it uncertain, not dismissible.
