Recommended Dose
300 mg standardized to 50% bacosides daily
Timing
Morning with a fatty meal for absorption
Mechanism of Action
Enhances synaptic communication via bacosides A and B, modulating serotonin, dopamine, and acetylcholine. Upregulates tryptophan hydroxylase and promotes dendritic branching.
Side Effects & Safety
Mild GI discomfort, nausea if taken on empty stomach. May cause drowsiness in some individuals. Onset of benefits takes 8-12 weeks.
Do not use / talk to a doctor first
- Bradycardia, or any condition worsened by increased cholinergic tone
- Gastrointestinal or urogenital tract obstruction, and peptic ulcer disease
- Asthma or chronic obstructive pulmonary disease
- Thyroid disease or on thyroid hormone medication

Cholinergic drugs (acetylcholine, methacholine, bethanechol)
Additive cholinergic effect - Bacopa has cholinergic properties and may increase the chance of side effects

Anticholinergic drugs
Bacopa may counteract their effect by inhibiting acetylcholinesterase and raising acetylcholine

Drugs metabolised by CYP1A2, CYP2C9, CYP2C19 or CYP3A4
Bacopa inhibits these enzymes in vitro, so co-administered drug levels can rise; a documented case of cholinergic toxicity followed exactly this route with cevimeline

Thyroid hormone medication (e.g. levothyroxine)
Animal work reports roughly a 40% rise in T4; use cautiously or avoid with thyroid medication

Sedatives
Bacopa is mildly sedating, so sedative effects may add up

Pregnancy & breastfeeding
Safety in pregnancy and breastfeeding has not been established. The StatPearls monograph does not address it, and no controlled human data exist - treat that as unknown rather than reassuring, and avoid.
Who should avoid / see a doctor
Anyone on thyroid, cholinergic or anticholinergic medication, on a drug metabolised by CYP3A4/2C19/2C9/1A2, or with asthma, COPD, peptic ulcer, bradycardia or a GI/urinary obstruction. Stop and see a doctor if you develop sweating, excess salivation, cramping or a slow heartbeat. Men trying to conceive should note that animal work reports reduced sperm count, motility and viability at higher doses.
