Why These Combine
The most expensive stack on this site, and the one with the least outcome evidence — which is worth saying plainly before the mechanisms. No compound here has been shown to extend lifespan or healthspan in a human trial. What NMN has shown is that it raises blood NAD+: in 80 healthy adults aged 40–65, 300, 600 or 900 mg a day for 60 days increased NAD+ at days 30 and 60, lengthened six-minute-walk distance against placebo, and left a blood-based biological-age estimate unchanged while the placebo group's rose; 600 mg was the dose at which NAD+ and walk distance peaked, and there were no safety signals up to 900 mg (Yi 2023). Two smaller trials at 250 mg a day — ten weeks in postmenopausal women with prediabetes (Yoshino 2021) and 6–12 weeks in healthy older men (Igarashi 2022) — found improved muscle insulin sensitivity and nominal gait-speed and grip changes respectively. Read those with two facts in view: none ran longer than 12 weeks, and several authors declared commercial interests — company employment in two of the trials, NMN patent interests in the third. PQQ at 20 mg has two Japanese 12-week trials reporting better memory and attention test scores in middle-aged and older adults (Shiojima 2022; Tamakoshi 2023), both small and both with company-affiliated authors; the mitochondrial-biogenesis story is animal and cell work. The CoQ10 trial cited here is about fatigue, not ageing — 300 mg for eight days in 17 volunteers (Mizuno 2008). The omega-3 slot rests on an association: in 608 patients with coronary disease, those with the lowest blood DHA+EPA had the fastest telomere shortening over five years (Farzaneh-Far 2010) — a cohort finding, not evidence that supplementing slows ageing. Vitamin D3 belongs here only to correct a deficiency, and the 5,000 IU listed exceeds the 4,000 IU adult upper intake level: test first. Compared with Clean Energy this keeps CoQ10 and adds the NAD+ and PQQ bets; compared with Mood & Balance it shares the omega-3. Verdict: if you want to spend on this category, NMN at 500–600 mg is the component that at least does what it claims biochemically; everything else here is either better placed in another stack or is a hypothesis you are funding.
Full Ingredient Protocol
Raises blood NAD+ reliably: 300–900 mg/day for 60 days in 80 healthy middle-aged adults, with longer six-minute-walk distance (Yi 2023). No human lifespan or healthspan outcome exists
Electron-transport cofactor. The human trial evidence here is a fatigue study (300 mg, 8 days, 17 volunteers — Mizuno 2008), not an ageing outcome
Two small 12-week trials at about 20 mg/day report better memory and attention scores in adults aged 20–80 (Shiojima 2022; Tamakoshi 2023); mitochondrial-biogenesis claims are preclinical
The telomere link is observational, in 608 heart-disease patients (Farzaneh-Far 2010) — an association, not a trial result
Correct a deficiency, nothing more: 5,000 IU/day exceeds the 4,000 IU adult upper intake level, so test 25(OH)D first (see the vitamin D3 page)
Disclaimer
NMN research is still evolving. Long-term human safety data is limited. Consult a longevity-focused physician before starting this protocol. Informational only — not medical advice. The human NMN trials cited here ran 12 weeks at most and several of their authors declared commercial interests; nothing here has been shown to extend life or delay ageing in people. 5,000 IU of vitamin D3 a day exceeds the 4,000 IU adult upper intake level — test 25(OH)D first, and do not combine high-dose vitamin D with thiazide diuretics or digoxin without medical advice. If you take warfarin or another vitamin-K antagonist, arrange INR monitoring with your doctor before adding CoQ10. NMN is not for anyone with active or past cancer without oncological advice. Not for pregnancy or breastfeeding.

- 1.Yi 2023, Geroscience — 80 healthy middle-aged adults, randomised multicentre double-blind, 60 days: NMN 300, 600 or 900 mg/day raised blood NAD+ at days 30 and 60 (p≤0.001); six-minute-walk distance increased in all NMN groups versus placebo (p<0.01), most at 600–900 mg; blood biological age rose in the placebo group and stayed flat on NMN; HOMA-IR unchanged; no safety issues up to 900 mg. (60-day trial; three authors, including the first, were employees of chemical companies (declared conflict of interest))
- 2.Yoshino 2021, Science — 10-week randomised double-blind trial in postmenopausal women with prediabetes who were overweight or obese: NMN increased insulin-stimulated glucose disposal and skeletal-muscle insulin signalling; placebo did not. (metabolic outcome in a specific population, not a healthy-adult or ageing outcome)
- 3.Igarashi 2022, NPJ Aging — Healthy older men, randomised double-blind: 250 mg NMN/day for 6 or 12 weeks raised blood NAD+ and NAD+ metabolites; nominally significant gait-speed and left-grip improvements that the authors say need validation; no effect on body composition; well tolerated. (three authors were employees of Mitsubishi Corporation Life Sciences (declared competing interest))
- 4.Mizuno 2008, Nutrition — 17 healthy volunteers, double-blind three-way crossover: CoQ10 300 mg/day for 8 days reduced subjective fatigue and preserved sprint velocity after 2 hours of cycling; 100 mg/day did not. (fatigue outcome, not ageing)
- 5.Shiojima 2022, J Am Nutr Assoc — 58 healthy Japanese adults (mean age ~71) completing a 12-week randomised double-blind trial: PQQ disodium salt 21.5 mg/day improved Cognitrax composite memory, verbal memory, reaction time, complex attention, cognitive flexibility, executive function and motor speed versus placebo; no adverse events. (small; several authors list company R&D affiliations)
- 6.Tamakoshi 2023, Food Funct — Double-blind placebo-controlled trial, adults aged 20–65: PQQ 20 mg/day for 12 weeks improved composite and verbal memory; younger adults (20–40) improved on cognitive flexibility and processing speed by week 8, older adults (41–65) on complex and verbal memory by week 12. (sample size not stated in the abstract; authors from Mitsubishi Gas Chemical)
- 7.Farzaneh-Far 2010, JAMA — Prospective cohort of 608 outpatients with stable coronary artery disease followed a median 6 years: those in the lowest quartile of blood DHA+EPA had the fastest leukocyte telomere shortening and the highest quartile the slowest (p<.001 for trend); each 1-SD rise in DHA+EPA was associated with 32% lower odds of telomere shortening. (observational association in heart-disease patients — not a supplementation trial)
