Why These Combine
Five compounds aimed at three different things — acetylcholine supply, cortisol control and cellular energy — for someone whose day is long, decision-heavy and stressful. The population mismatch that runs through this whole site is largest here, so take the trials one at a time. Citicoline at 500 mg is the dose from the best healthy-adult trial available: 100 adults aged 50–85 with age-associated memory impairment improved on episodic and composite memory after 12 weeks against placebo (Nakazaki 2021), and 250–500 mg a day improved attention and psychomotor speed in 75 healthy adolescent males over 28 days (McGlade 2019); the Focus & Flow stack uses the lower dose. Ashwagandha's cortisol evidence is the strongest fit for the brief: 300 mg twice daily for 60 days lowered every stress scale and serum cortisol in 64 chronically stressed adults (Chandrasekhar 2012), and 240 mg a day lowered anxiety ratings and morning cortisol in 60 stressed healthy adults (Lopresti 2019). Magnesium L-threonate is the component whose marketing has run furthest ahead of its data. The brain-magnesium and synapse-density claims come from the developers' own rodent work, as the trial paper itself states; the one placebo-controlled human trial randomised 44 adults aged 50–70 with cognitive impairment to MMFS-01 — a magnesium L-threonate compound trademarked as ClariMem, tested by the company that developed it — or placebo for 12 weeks and found a composite cognitive score improved with a large effect size (d=0.91), while sleep and anxiety outcomes could not be judged because of placebo effects (Liu 2016). A 30-day trial in 109 healthy Chinese adults used a combined threonate-plus-phosphatidylserine-plus-vitamins formula, so it cannot say what the magnesium did on its own (Zhang 2022). ALCAR's meta-analysis benefit — effect size 0.20 — is in mild cognitive impairment and early Alzheimer's at 1.5–3 g a day (Montgomery 2003), three to six times the 500 mg here, and a randomised trial in women on taxane chemotherapy found it made nerve damage worse (Hershman 2018). Omega-3 at this dose has a healthy-young-adult trial behind it, but only in people whose intake was low (Stonehouse 2013). Compared with Focus & Flow this is heavier, costlier and adds cortisol and magnesium components; compared with Stress Shield it keeps the ashwagandha and adds cholinergic and energy compounds; Memory & Recall is the better choice if recall rather than stamina is the complaint. Verdict: citicoline and ashwagandha earn their places; the threonate is a reasonable trial if you accept that its evidence is one small study in impaired adults; ALCAR is under-dosed relative to its own evidence and carries a real caution — and the existing advice to introduce one compound at a time is the most important line on this page.
Full Ingredient Protocol
500 mg/day for 12 weeks improved episodic memory in 100 adults aged 50–85 with age-associated memory impairment (Nakazaki 2021); 250–500 mg improved attention in healthy adolescents (McGlade 2019)
600 mg/day (as 300 mg twice) for 60 days lowered stress scores and serum cortisol in 64 chronically stressed adults (Chandrasekhar 2012)
The one placebo-controlled trial — 44 adults aged 50–70 with cognitive impairment, 12 weeks — improved a composite cognitive score, d=0.91 (Liu 2016); the brain-targeting claim comes from the developers' rodent work
Meta-analysis benefit is in mild cognitive impairment and early Alzheimer's at 1.5–3 g/day (Montgomery 2003) — three to six times this dose
1.16 g DHA/day for 6 months improved memory reaction times in 176 young adults with low habitual DHA intake (Stonehouse 2013)
Disclaimer
This is a comprehensive stack. Introduce one supplement at a time over 2 weeks to monitor individual responses. Consult your physician if on any medications. Informational only — not medical advice. Do not take the ALCAR if you are receiving taxane chemotherapy — a randomised trial found it made nerve damage worse; ALCAR also interacts with warfarin and thyroid medication. Ashwagandha may interact with thyroid medication. Magnesium L-threonate is still magnesium: avoid it with kidney disease, separate it from tetracycline or quinolone antibiotics, and take it at least two hours apart from oral bisphosphonates. Ashwagandha and ALCAR are not for pregnancy or breastfeeding, and citicoline and the threonate form are not adequately studied in pregnancy — agree any supplement with a midwife or doctor.

- 1.Nakazaki 2021, J Nutr — 100 healthy adults aged 50–85 with age-associated memory impairment, randomised double-blind: citicoline 500 mg/day for 12 weeks improved episodic memory (paired-associate test 0.15 vs 0.06, p=0.0025) and composite memory (3.78 vs 0.72, p=0.0052) versus placebo.
- 2.McGlade 2019, J Atten Disord — 75 healthy adolescent males randomised to 250 or 500 mg citicoline or placebo for 28 days: improved attention (p=0.02) and psychomotor speed (p=0.03).
- 3.Chandrasekhar 2012, Indian J Psychol Med — 64 adults with chronic stress, randomised double-blind: 300 mg full-spectrum ashwagandha root extract twice daily for 60 days reduced all stress-scale scores (p<0.0001) and serum cortisol (p=0.0006) versus placebo.
- 4.Lopresti 2019, Medicine (Baltimore) — 60 stressed healthy adults, 60 days: 240 mg standardised ashwagandha extract once daily reduced Hamilton Anxiety scores (p=.040) and morning cortisol (p<.001) versus placebo; no adverse events.
- 5.Liu 2016, J Alzheimers Dis — 44 adults aged 50–70 with cognitive impairment, randomised double-blind: MMFS-01 (a magnesium L-threonate compound, n=23) vs placebo (n=21) for 12 weeks improved overall cognitive ability (p=0.003, Cohen's d=0.91) and reduced cognitive fluctuation; effects on sleep and anxiety could not be determined because of strong placebo effects. (small, impaired population, run by the compound's developer (Neurocentria); the sole placebo-controlled trial of this form)
- 6.Zhang 2022, Nutrients — 109 healthy Chinese adults aged 18–65, double-blind: 2 g/day of a magnesium L-threonate + phosphatidylserine + vitamins C and D formula for 30 days improved all five Clinical Memory Test subscores versus placebo, more so in older participants. (combined formula — cannot isolate the magnesium effect)
- 7.Montgomery 2003, Int Clin Psychopharmacol — Meta-analysis of double-blind placebo-controlled trials of 3–12 months at 1.5–3.0 g/day in mild cognitive impairment and early Alzheimer's disease: integrated effect size 0.201 (95% CI 0.107–0.295); well tolerated. (clinical populations at three to six times the dose used here)
- 8.Hershman 2018, J Natl Cancer Inst — SWOG S0715: 409 women on adjuvant taxane chemotherapy, ALCAR 1000 mg three times daily vs placebo for 24 weeks — significantly worse neuropathy scores (−1.39 points, 95% CI −2.48 to −0.30, p=.01), still evident at two years. (randomised trial showing harm in a specific population — the basis of the chemotherapy warning)
- 9.Stonehouse 2013, Am J Clin Nutr — 176 healthy adults aged 18–45 with low habitual DHA intake: 1.16 g DHA/day for 6 months improved reaction times of episodic and working memory versus placebo.
